The hospital uses EmmiTransition—an automated, interactive system of outreach tools—to call patients for 45 straight days after discharge, reminding them when to schedule an appointment with their primary care doctor, take their medication, or complete other actions.

Medicare patients are getting sicker and older. That was the takeaway message of an AHA TrendWatch report published in December 2012. The American Hospital Association found that in 2008, two-thirds of all Medicare beneficiaries had at least two chronic conditions, and this number is expected to increase. Furthermore, overall healthcare spending for someone with one chronic condition is nearly three times more than for someone without any chronic conditions, and approximately 17 times greater for someone with five or more chronic conditions, according to the report.

Like most hospitals around the country, Hackensack (N.J.) University Medical Center is focusing on reducing readmissions. As part of this effort, the hospital was among four hospitals to participate in a pilot of a new program designed to follow up with patients after discharge to prevent readmissions. Overall, hospital staff at the pilot sites saved an estimated 600 hours, and the hospital found more than 313 "red flags" — conditions that impact patient outcomes, satisfaction and risk.

"Although the numbers are small and this wasn't a scientific project, there's been a marked reduction in readmission rates for all causes for those patients enrolled in the program." Teichholz said the program's readmission rate has gone from "20% in general" to under about 5 or 6%. "We think this has been helpful, and it's allowed us to do it with our resources--those are always limited. This helped us improve interactions with patients. One of the things with readmission is patient understanding, interaction, and education. We think this helped."
